Yess, Ghost will be a she to match Arra. There are few reasons she left her behind: 1) she wanted to protect her incase Targaryens wanted to harm her, 2) Targaryens would not agree such a strong creature be amongst them (she is a captive afterall), and 3) she wanted Ghost to remain with her brother Grey Wind unlike herself who had to leave her brother.
And yes, you are correct about Rhaegar only seeing pain in Arra coz she remind him of Lyanna.
