Interesting concept I look forward to see where you go with this. That being said why does midoriya not have a sense of taste or smell? Almost every single sea creature that I know of has a very good sense of taste and or smell sharks come to mind at one of the as one of the best. Also his body is able to draw the salt out of the salt water to keep his blood sodium levels high enough why can't his body draw the water out of the sea water to keep them from dehydrating ? lots of fish and sea mammals not to mention sea birds do something quite similar. Mind you keep birds and sea mammals tend to get rid of the excess salt in one way or another as well.
Despite Izuku's mutations he is still human and still warmblooded. In my research I found several articles about divers who stay under for long periods lose these senses. And Izuku stays in the water for a looong time, very often. He can't draw the water from the sea water because he is breathing it when he is in the water. His body can't do both at the same time. The salt is just a happenstance, it is not what the gills are meant to be breathing. His chronic dehydration is a result of his hydrokinesis (the more he uses it the worse it is, depending on how he is using it) ,while the hyponatremia is directly part of his mutation. If he didn't have it he would get really sick from being in the saltwater so much. It's what allows him to spend so much time in the water.
