I realize it is not 100% clear in chapter 1, but uh...Inko and Hisashi died in said accident. And Izuku DID get therapy, he just never really absorbed it is the problem. A therapist can only help someone who lets themselves be helped, and he only kind of has. Or rather, intellectually, he knows everyone around him is right and he's not at fault, but the heart doesn't always listen to the brain. I am drawing on some of my own experiences when I was younger for that sort of mentality heh...a parent can do everything right and the kid can still not accept it in their heart unfortunately.
