Primus was all knowing maybe, but just like he said, he wasn’t all powerful. Primus couldn’t send him back. But Sam could.
Worst case scenario, he evaporates into nothingness, but at least that would be better than spending an eternity here, watching everyone he loves die, watching as another version of himself enjoys everything he had been fighting for.
Was it dramatic? Probably. But Sam was young, he was meant to be dramatic. And for his family, he’s done crazier things.
Or: Sam gets transported to the Transformers Prime universe and ends up messing up fate's plans for its inhabitants, for the better, or for the worse.

After the events of Revenge of the Fallen, without something to focus on, Sam finds himself overcome and overwhelmed by the All-Spark. The voices and the symbols are too much for his human mind to handle. The All-Spark killed Megatron, and it's slowly killing him. Between seizures and blackouts, Sam is losing his mind. He's losing himself to the all of the All-Spark. He falls away when the voices take over and when he comes too, a new half-crazed project is laid out before him, and he doesn't remember any of it. Mikaela tries to help by lending her mechanical skills to said projects laying around the NEST base and keeping Sam as on track as he can be.
Sam realizes, almost immediately, that he's only ever himself when he's with Optimus Prime, due to the Autobot carrying the Matrix of Leadership, the ancient artifact gifted to Sam by the Primes of old. The two spend as much time together as they can. Bumblebee, too terrified of losing Sam again, is always following him, and only ever leaves if he's sure Sam is safe.

Then comes the Ark, Sentinel Prime, and murder. And most of NEST thinks Sam is nuts, even though he's not.[A rewrite of Dark of the Moon starting directly after Revenge of the Fallen.]

Not risking any more near-death experiences with the former Allspark holder though remaining in touch as friends. Sam tries to move on and live a normal life but due to his relationships with the Autobots his life will never be the same. It gets even weirder and complicated when a certain yellow Autobot reveals a secret to him that will definitely change his life forever.
