When Stiles' Great Aunt found herself in possession of a baby following the death of her nephew, she was at a loss. What should she do with a rare male Omega? Not one to stand for ambiguity, she raised Stiles as a girl. And to that end conspired that Stiles should marry the son of one of her fondest friends, future Duke Alpha Derek Hale. Stiles loved Derek and his cousin Scott dearly as she grew up alongside them. As a child, they were her knights in shining armor and the source of endless piggyback rides. As she grew, her feelings changed from sisterly affection to something more. After catching the two Alphas in a compromising position, she realized that Derek had no interest in women, which caused her endless despair even after she and Derek were married.
When Derek finally summoned up the gumption to consummate his marriage (with a scandalous amount of assistance from Scott), he and his cousin are both surprised to learn that his wife has a secret. One that she keeps under her skirts and didn't even know she was keeping!

Noah winced, unable to stop his eyes from darting to the small pile of papers he’d brought back with him. Stiles’ looked as well, hand darting out as if to pick up what was on the top only to freeze, fingertips less than an inch away from ‘Advanced Maternal Age Pregnancy in Omegas’, typed in green block letters right above a glossy picture of an older looking omega with graying hair, laugh lines, and a prominent, rounded stomach. It was a pamphlet that Noah hadn’t even cracked open, vaguely annoyed at the title even through the haze that had wrapped around his mind.
“What the fuck?” Stiles breathed.
Noah closed his eyes and pinched the bridge of his nose. “Stiles, at least shut the door before you freak out.”

Noah thinks he's too old and set in his ways for more children, but the result of a frantic, post battle fling with Peter Hale proves that his body, at least, disagrees.
