“If you don’t want me to go with you...then don’t go, Dean,” the ex-angel finishes with a note of casualness in his voice, as if the decision is simple.
Unfortunately, Dean Winchester is nothing if not stubborn.
He sucks on his teeth for a moment, roughly picks up his duffel, and slings it over his shoulder as he storms past Cas in the doorway with a bite of, “Go get in the damn car,”.
(TLDR: Cas is rescued from the empty, human. Dean wants to stop hunting because he loves Cas, but can't figure out how to let himself; however, he doesn't want Cas to hunt either, because he loves him. Conflict, angstiness, and love ensues.)

Summary
Dean is not okay, and he doesn’t try to hide it anymore. He wants to bring Cas back, and he won’t let anything stop him. He’s willing to threaten, to hurt — others or himself. He pushes away the people who care about him, convinced his toxic presence will ruin their new, free lives. But they refuse to let him go.
Meanwhile, Castiel is awake in the Empty. Something is keeping him from rest. The last remnants of his angelic grace reach for the only piece of him left on Earth — a piece that is broken, shattered, but still longing for him.
This is a story of second chances, of fighting your demons, and finally allowing yourself to be free. The brothers set out on one last hunt, ready to claim the life they never thought possible.

He watches Sam’s finger point to the written words, “Dean still hardly talks,”.
A few beats of silence pass—nothing out of the ordinary, at this point.
“...Is this what’s happening?” Sam asks gently, implementing that tone of voice that always seems to remind Dean with a heart clenched in nostalgia that his brother is not a little boy anymore, but rather is now a grown man with an impressive emotional maturity.
(TLDR: Dean Winchester and selective mutism: told throughout different stages of his life now that it has come back in full-force yet again after losing Cas to the Empty. Angst, hurt/comfort, and most importantly: happy ending.)
